ROLE & RESPONSIBILITIES

I. PRIMARY FUNCTION:

To ensure that the service member receives appropriate, effective, and efficient services, allowing the service recipient to retain or achieve their optimal level of independence.

To provide support services to individuals to assist them in acquiring and developing skills in self help in daily living activities, leisure and recreation activities, communication and socialization skills.


II. RESPONSIBLE TO: Area Manager

POSITION SUPERVISED: NONE


III. RESPONSIBILITIES:

  • Assists individuals in training activities as specified.
  • Assists and trains individuals in performing daily living functions such as bathing, brushing hair and teeth, dressing, toileting, and other areas that require individual personal assistance.
  • Monitors individuals’ activities to assure safety and health care practices are observed.
  • Ensures consistent implementation of any approved plans.
  • Transports individuals on community outings and as needed, to other programs or appointments.
  • Provide accurate documentation of individual’s progress and participation in all daily living skills, community outings, medical visits, etc.
  • Assists in coordinating and conducting individual and group recreation
  • activities.
  • Reports promptly all instances of abuse, neglect, and exploitation using procedure as specified in the individual abuse policy and procedure.
  • Informs supervisor of any individual illness or injury and documents such occurrence using the appropriate Incident report as specified in the policy and procedure.
  • Demonstrates positive interest in working with and relating to individuals and their families by demonstrating positive communication and interaction activities.
  • Performs assigned housekeeping duties to ensure the cleanliness of the home is maintained.
  • Provides housekeeping training and assistance which includes, but not limited to changing linens, cleaning individual areas and equipment and
  • making sure individual clothing gets washed, mended, and folded.
  • Lifts/moves non-ambulatory individuals using the correct lifting procedure as specified by the trained plans/programs.
  • Completes daily observation sheets, log entries, daily reports and program data forms.
  • Communicates regularly with the Area Manager and other appropriate staff members regarding individual programs and progress.
  • Attends regularly offered in-services and demonstrated learned competencies.
  • Understands and adheres to policies and procedures.
  • Performs other duties assigned.
  • Be knowledgeable of different cultural backgrounds.
  • Be willing to submit and pass a criminal background check


IV. QUALIFICATIONS:

  • High School Diploma or GED equivalency required.
  • Must not be averse to working in close physical contact with people or providing personal hygiene assistance to individuals who cannot provide for themselves. 
  • Must have compassion, patience, and ability to nurture towards older persons and persons with disabilities.
  • Possession of a valid state driver’s license along with an acceptable driving record.
  • Verifiable work background along with proven reliability is required.
  • Must be able to safely lift and perform the physical characteristics of the job with or without reasonable accommodation.
  • Must attend and complete all training requirements including First Aid, and CPR.
  • Must not be a member of an enrollee’s family.
